Resources
For DVA Members:
A GP (Doctor’s) Referral is required. Please bring DVA Card on first visit. We can treat GOLD or WHITE card holders.
For WORKCOVER claims:
Current Workcover medical certificate: covering the period required for Physiotherapy treatment and specifically requesting Physiotherapy
Claim Number: if known (If Claim Number Pending - you can commence treatment at your own cost and be reimbursed by Workcover)
Case Manager name and contact details: if known
For EPC / GP Management Plan patients:
Medicare card and debit card for Medicare instant rebate processing onsite.
Enhanced Primary Care (EPC) / Team Care Arrangement (TCA) from your GP Plan: paperwork needed (if not already provided to us by GP).
GP and relevant medical specialist details: To complete our patient registration form, please have these details on-hand.
For INSURANCE / CTP Claims:
If you have been in a Motor Vehicle accident and require Physiotherapy we can help you. You will need the following:
- Your insurers details
- Claim number
- Case Manager details
- Medical Certificate (if required by insurer)
- Fees can be billed directly to the Insurance Company
Fees Policy
- Fees are to be paid at the time of consultation.
- Different fees apply for Specialised consultations e.g. NDIS, Insurance, Psychology & Counselling services.
- We will bill HICAPS participating Private Health funds directly and you may pay the Gap by cash or card.
- If you are unreasonably late for an appointment, we reserve the right to change it to another time so that other patients do not have to wait.
- We require 24 hours’ notice for cancellations otherwise our Late Cancellation fee will be charged.
- *Senior Tier 1, Tier 2, Tier 3, Tier 4 & Principal is based on years of experience.
